- Copy of social security card
- Proof of insurance for state sent from your Insurance company with NRI as additional insured/certificate holder
- Commercial liability
- Automobile insurance
- Workers comp
- W9
- Valid business license per state
- Bank statement/number
- Signature for legality of employees
- Number of employees
- Location of employees
- Project references (5)
- List of other work being performed while working for NRI
